I understand that the Football Association, Stalybridge Celtic Juniors and their agents are not liable in any way in respect to personal injury, loss or damage, however caused whilst attending Soccer School.

I understand that all players must wear shin pads, appropriate footwear and appropriate clothing for the conditions at all times.

I consent to Stalybridge Celtic Juniors FC photographing or filming the involvement of the above Child whilst playing at Soccer School and use these images on their social media channels. I can confirm that this child is not subject to family, care or legal proceedings.
